The next-generation primary care experience

Three clickable hypotheses about the clinician and patient experience of an AI-native clinic, each paired with a working mockup.

Inspired by Transforming the Patient-Centered Medical Home · J Gen Intern Med, 2026

Hypothesis 01

Patient time is no longer rationed. AI gives every patient unlimited, around-the-clock attention, so continuous care is no longer a luxury.

Hypothesis 02

Human judgement is the scarce resource. Clinical reasoning is now abundant. A clinician's judgement and the relationship are what stay scarce.

Hypothesis 03

AI catches what no rushed clinician has time for. The OS surfaces the risk early, for a physician to see and own.

Hypothesis 01 · patient + clinician surfaces
Care becomes continuous, not episodic
A 24/7 front door makes the visit a checkpoint in an ongoing thread, not the only point of contact.
Look for: trace any brief line back to the patient's words, plus the English/Español toggle.
Hypothesis 02 · clinician surface
The clinician's job becomes judgement and relationship building
AI drafts the reasoning, notes, and triage; the day narrows to the calls that need a human.
Look for: "needs you, not booked" up top, a thread per patient, and Start visit → Presence → drafts routed to oversight.
Hypothesis 03 · system surface · the centerpiece
Liability becomes a moat, not a barrier
AI catches the subtle risks a human-only practice misses, and proves every catch.
Look for: the dual lane of "AI acted" and "AI held back," and risk-tiered sign-off.
